The ASD has now moved to Final Forms to clear athletes to participate in sports, you should have received a Parent Square message from the district on 10/30 with instructions and link to Final Forms.
Please log into Final Forms with your parent email. Choose the sport(s) your student is participating in and fill out the required forms.
*Winter Season Sports Starts Nov 17th – (Boys & Girls Basketball, Dance, Boys Swim/Dive, Boys & Girls Wrestling) *Gymnastics starts Nov 10th
Athletic clearance MUST happen on Final Forms for your student to be able to participate.
If your students information is not in Final Forms, or for any questions, please contact the Athletics & Activities Office for assistance 253-804-5159.
